We offer Case Management services with a personal touch. I am an RN with 30+ years of nursing experience. Our goal is to keep you in your home for as long as possible, by utilizing services provided by the Medicaid Long Term Waiver. The Long Term  Waiver includes the Community Choices Waiver and the Assisted Living Waiver. 
The Community Choices Waiver (CCW), offers: In home services  for individuals who need a nursing home level of care, but wish to remain in their home and receive specialized services that could delay or prevent admission to a nursing home.  Some of the services offered are; 
*Home delivered meals.
*Personal emergency response system.
*Light housekeeping
*Personal Care Attendants
* Skilled Nursing services.
* Non-medical transportation.
Self Directed Care Waiver: You hire, train and monitor your caregiver. This is a specialized service that some qualify for.  Access pays the caregiver. This is not a full time position and no benefits are provided.
The Assisted Living Facility Services offers an option for individuals who need a nursing home level of care, but wish to remain in the community in an assisted living facility and receive specialized services that may delay or prevent admission to a nursing home. 
 Medicaid offers assistance in paying medical bills, prescription medications and the above listed services.
